  Uncommitted type (狭義のGIST)  

GISTは主に上部消化管(60-70%が胃、20-30%が小腸、他に大腸、食道がそれぞれ5%程度)にみられる間葉系腫瘍で、20/100万人ほどの発症率。発症は主に40歳で50-60歳代が最も高い。一般に、免疫組織化学的に平滑筋としての分化マーカーの発現がみられるものは筋原性腫瘍、S100など神経系マーカーの発現がみられるものは神経鞘腫として扱われ、両者の発現がみられないもの(Uncommitted type )が狭義の意味でのGISTとされる。狭義のGISTではCD34やc-kitの発現率が高く、特にチロシンキナーゼ型受容体であるc-kitの発現する症例では遺伝子の変異によるシグナル伝達の増強が腫瘍細胞の増殖を強く促すと考えられており、化学療法や放射線療法に強い抵抗性を示すことが知られているが 現在、「分子標的治療」としてシグナル伝達系を阻害する治療薬が承認されている。
